eConsent 'Consent to Treatment' Form Generator

Informed, standardised patient consent

The eConsent project set out to solve the issues caused by traditional, handwritten patient 'Consent to Treatment' forms. Developed at Worcester NHS Trust, the system gives a clinician access to a tailored library of consent forms via a tablet device or desktop computer. These are pre-populated with benefits, risks and linked to appropriate PILs. Full patient information is automatically included from the PAS and the clinician can add, remove or re-prioritise key points before producing the final printed copy and archiving an electronic version.

About

  • Consent forms being completed by inappropriate members of the clinical team? eConsent only gives the user access to forms appropriate to their seniority and speciality, with an option to "favourite" those in common use.
  • Patient information missing or incorrect? Full patient data is taken from the PAS and included on the form. The form is dated automatically where appropriate.
  • Clinical guidance points missing or abbreviated? The clinical user is presented with a full list of benefits and risks as agreed at a Trust level, but has the ability add, remove or re-prioritise the list as they see fit. All clinical terms are described in full.
  • Appropriate Patient Information Leaflets difficult to locate? Any relevant PILs can be printed out alongside the form.

An eConsent smart paper form provides the clinician with a fast and efficient way of explaining the procedure, the patient a clear and complete picture of what they are consenting to and the Trust with documentary evidence of a structured discussion about the procedure in case of litigation.
