KSS AHSN Alliance for AF

The KSS AHSN is working to reduce the number of people dying or disabled by AF related stroke by optimising the use of anticoagulants.

About

We have interventions to deliver impact at various stages of patients diagnosis and treatment of AF:

  • Detect – Increase the prevalence of AF: where success is driving up numbers of people diagnosed with AF
  • Review  - Provide timely reviews: where success is driving up the % of people diagnosed with AF being prescribed anticoagulants
  • Protect  - Increase optimal anticoagulation: where success is driving up the % of anticoagulated patients receiving optimal anticoagulation

The proposed KSS AHSN Alliance for AF project has two elements:

  • The AF virtual menu (that will provide devices, services, information to compliment and improve existing GP services)
  • The AF dashboard will be developed by the AHSN (that will include all National datasets drilled down at CCG and Practice level and be updated quarterly and also be used as an improvement monitoring tool to report on impact / outcomes)
