Lancashire Care NHS Foundation Trust (LCFT), in collaboration with Healthcare at Home, Midland Heart and Preston Primary Care has created a data system to provide a safe alternative to a hospital inpatient stay.
The data sharing system and care record delivers benefits to the patient and the health economy, including:
- seamless transitions through the health service
- up to date information supporting sound clinical judgements
- the means for patients to be treated and cared for at home
- a dedicated phone line for patients and families to ring for advice and support.
The work was shortlisted for the Enhancing Care by Sharing Data and Information category at the 2015 Health Service Journal Awards.
