Casper: primary care pathway orchestration

Casper is a web-based platform that captures information from clinicians in an intelligent way, asking them questions so that all necessary information is captured in one place.

Casper uses logic to make suggestions to the clinician, triaging their patient, guiding them through the process in order to get the patient to the right place at the right time, according to protocol.

Information is made available to all appropriate users on the platform in real time. Casper also manages workflow, prompting the right users (clinical/admin) to contribute to the referral and progress it.

Let's say, for example, that a GP has a consultation with a patient who has symptoms suggestive of ankylosing spondylitis. The GP can refer the patient to rheumatology for a specialist opinion using Casper.

To create the referral, the GP submits information on the Casper form. Any answer may trigger further questions, depending on the logic. For example, if the GP says that the patient is having treatment x, the form will ask them additional questions which are pertinent to treatment x. This ensures that all necessary information is captured but no irrelevant questions are asked.

Casper also uses rules to validate whether the referral is clinically appropriate. This provides immediate feedback to the GP as to whether their referral is appropriate or whether other investigations could/should be undertaken in primary care instead. This could include additional diagnostic tests or more in-depth clinical history.

Once the GP has made the referral, Casper then automatically updates the GP regarding the outcomes of the referral. This can be as detailed as local need dictates and can be in real time or as a summary care record. For example, Casper can alert the GP as to when the patient has an appointment booked with a rheumatologist, the outcome of this appointment, any diagnostic tests undertaken, any diagnosis made, treatment plan and more.
